We have a number of personalised services for people with learning disabilities which include supported living and outreach support in the wider community.
We promote individuality through developing and maximising peoples abilities, offering opportunity, encouraging positive engagement, providing community resources, and encouraging people to take control of their individuality.
We are provide outreach services to people living in their own homes, irrespective of their disability or sometimes socially challenging behaviours. This is achieved by understanding the current and anticipated needs of the induvidual, by undertaking continuous appraisal and on going assessment of their progress.
Every one of our services is focused on Person Centred Working. This means that the support we provide has the person at the centre, the focus of our approach is on what is important to them. We believe that these person centred plans give us a firm foundation for support; enabling us to continually listen and learn about what is important to an induvidual now and in the future, and to act on this in partnership with them, their family and/or representatives.
