We promote individuality through developing and maximising peoples abilities, offering opportunity, encouraging positive engagement, providing community resources, and encouraging people to take control of their individuality.
We provide outreach services to people living in their own homes, irrespective of their disability or sometimes socially challenging behaviours.
This is achieved by understanding the current and anticipated needs of the people, by under taking, continuous appraisal, and on going assessment of there progress, in order to be able to plan with them, their family, care managers and other agencies.
• To work closely with other agencies involved with the person, relatives, care managers, health professionals and benefit agencies.
• To identify, implement and review personal centered plans which reflects the needs of the individual
• To promote independent living – working within the range of the individuals abilities
• To insure the health needs of the individual are met.
• To attend to the safety and security of the individual and their home.
• To improve and increase the involvement of the individual in the community.
• To enable the individual to increase their life skills with the hope that they can move on to independent living in the future.
